Blue Dot Intake Script

What “Blue Dot” Means Any client marked as a Blue Dot has been referred to us through the FBI, HSI, Black and Missing, or NECMEC. These clients are victims of crime and often come to us with unique safety and confidentiality considerations.

For Admins If you receive a referral directly from one of these partners, please follow the proper intake script and be sure to add the unique identifier “Blue Dot” to the client’s file. This helps our team maintain the highest standard of care and compliance. Hello, and thank you for calling Lartey Wellness Group. My name is _______. I’ll be assisting you today to help connect you with the right therapist and services. It takes courage to take this first step, and I want to assure you that we’re here to listen, support, and connect you with the care you deserve. In order to get started, I have a few questions to ask, just to ensure we provide you with the most appropriate care and support.

Were you referred to our therapy services by another agency or organization?

● If No: That’s okay; I’ll still do my best to assist you.

â—Ź If Yes: Continue with the next section of the script.

Can you please provide me with the name of the organization the referred you?

· If FBI, HIS, NECMEC, Araminta, Black and Missing Foundation or ICAC, please make the client as a “Blue Dot” and proceed.

· If no agency is mentioned, proceed without marking the client as a Blue Dot.

Thank you for letting me know. I’m so glad you found us, and I want to assure you that we’re here to help you every step of the way. I’m going to do everything I can to get you connected to the right therapist as soon as possible. In order to get started, I’ll need to collect your information. Some of our intake questions may feel personal, and I want to ensure that you are in a private place to begin. Before we continue, may I ask if you’re in a safe place where you can speak freely?

â—Ź If Yes: Proceed.

● If No: Thank you for letting me know. Would you like me to schedule a call-back at a time and manner that’s safe for you? If there are precautions, we should take, please let me know.

To ensure I understand your needs fully, I have a few more questions:

1. Are you seeking therapy services for yourself, or are you inquiring on behalf of a loved one (or both)?

â—Ź This helps us understand who the primary client is and ensures we provide the appropriate support.

2. Are you interested in individual therapy, family therapy, or both?

â—Ź This will help us match you with the right type of service.

3. In addition to traditional therapy, we offer mentorship services that could be very helpful, especially if you need assistance with errands or social activities. For younger clients, we have a unique program where a mentor takes them to Urban Air, an indoor theme park, providing both fun and supervision in a safe, structured environment. Does this sound like something you or your loved one might be interested in?

Can you share a little about your most pressing needs or challenges you’re currently facing?

â—Ź Mentorship can be a vital part of the healing and recovery process.

4. We also offer a service called PRP alongside therapy, which provides additional, more intensive support to help you with daily life challenges, recovery, and goal-setting. Would you be interested in hearing more about our PRP services?

â—Ź This could be especially valuable if you or the person you are calling for needs more comprehensive care and guidance.

Thank you for sharing this information with me. I want to make sure we are tailoring our services to meet your specific needs and provide the kind of support that will be most beneficial for you.

5. What type of insurance do you have? Can you please read me the group number and the policy number?

Thank you again for that information. We’re about halfway through.

Before we proceed with scheduling, I’ll need some additional information:

â—Ź Can I please get your full name and date of birth?

â—Ź What is your preferred contact information (phone number, email)?

â—Ź Is it okay if your therapist leaves messages at the phone number you provided?

â—Ź Will it be alright for your therapist to reach out via text, if necessary?

â—Ź Are there any urgent mental health or safety concerns that we should be aware of as we connect you with services?

This helps us prioritize cases if there is an immediate need for assistance.

â—Ź Have you or your loved one recently experienced a situation where you felt unsafe or threatened by someone else?

â—Ź Have you experienced or witnessed any acts of violence that have impacted your sense of safety?

Make sure to give the client time to fully explain.

Thank you for sharing this information with me. I want you to know that we take confidentiality and safety very seriously. All information shared with us will remain private and will only be used to connect you with the right services and support.

I will now work on matching you with a therapist who is best suited to your unique needs.

â—Ź Do you prefer a male or female therapist?

â—Ź Do you prefer a certain age bracket for a therapist?

â—Ź Is there anything else we should know, with respect to your ideal therapist?

â—Ź Do you prefer a therapist who can meet you in person, virtually, or both?

â—Ź Do you prefer a specific kind of therapy like trauma-focused work, humanistic, or CBT?

â—Ź How many times per week would you ideally like to meet with your therapist?

â—Ź What is the best day and time for you to meet with your therapist?

We’ll do the best we can to match you with a therapist who meets as many of your preferences as we can. Please know that we understand the unique challenges you may be facing, and we are here to provide you with the care, respect, and resources you deserve.

Is there anything else you’d like to ask or share with me at this time?

â—Ź This gives the caller an opportunity to voice any additional concerns.

Thank you for all you’ve shared with me. Also, thank you for being patient and hanging in there, through all the questions I needed to ask you. So, let me explain what will happen now. Someone will contact you shortly to confirm your first appointment with your Lartey Therapist. Thank you for reaching out to Lartey Wellness Group and know that you are not alone on this journey.
