Transition Plan

A transition plan is a strategy to facilitate a smooth change from one phase of care to another, whether it's from outpatient care or when a client moves to a new therapist or practice. Key components include detailed communication, coordinating record transfers, scheduling follow-up care, providing education on risks and coping strategies, and building supportive networks with family, friends, and community resources. The plan is tailored to the individual's needs, incorporating their participation and addressing potential disruptions to minimize stress and support continued stability and independence. CARF recommends that we complete transition plans at the start of services and update them throughout the course of treatment and during termination.

The transition plan is a TherapyNotes portal form to be shared within the client's portal. The client is to complete the transition plan within or after a session. Please note that therapists will not be able to complete a transition plan since the document is a TherapyNotes portal form. TherapyNotes portal forms can only be completed by clients. After completion, therapists should review the transition plan with the client during a therapy session.
