How much will I be compensated?

Each clinician will be compensated $50. Therefore, it is important that the $100 fee is collected.

How will I receive my compensation?

It will be included in your pay every payroll period. Please make note of STEP 3 in the process above.

How will you keep track of the number of forms/letters I have completed?

We do daily audits of each clinician's calendars and the copays [to be] collected.

How and where do I collect payment for this service?
